Preschool Application

Application for IMACA Preschool Program

This application is not complete without the following:

  1. Birth certificate for each child under the age of 18 in the home
  2. Immunization record with physical and TB for enrolling child
  3. Full month of income for the last 30 days for the parent(s) in the household
  4. Click here for the well-child check/physical form to take to your child’s doctor.
  5. Click here for the Employment Verification form to be signed by your employer.
