Volunteer Agreement

I agree to follow all the policies and procedures outlined in the IMACA volunteer handbook. I understand that my relationship withIMACA is strictly voluntary and that either I or IMACA may end this voluntary relationship at any time.

agree that as a Volunteer I will hold in confidence all personal and private information concerning all clients and staff of the Inyo Mono Advocates for Community Action AgencyI will hold this information in confidence while I am associated with IMACA and after I terminatemy employment with lMACA.

I will never disclose any information about a client or a client’s family that I learned from IMACA work or files.

I will never disclose any information about a client or a client’s family that I observed during a home or site visit to anyone but appropriate IMACA administrators.

I will never disclose any information about a client or a client’s family that I heard at a staff meeting, conference, or informal discussion.

I will only discuss a client’s case in general terms (not using names) as an example in a situation having an instructional value, such as: Reports or term papers in academic courses; To help a staff person/volunteer, become oriented to the IMACA program.

I will never disclose any information about other IMACA employees that I learn from their private files, records, private conferences, or conversations.

I will only discuss a fellow employee’s professional behavior or work performance with the employee’s superiors within the IMACA staff structure.

I will hold in confidence all personal and private information concerning the staff and clients of any agency I am associated with through my position at IMACA. I will hold this information in confidence while I am associated with IMACA and after I terminate my employment at IMACA. I will follow the grievance procedure as established herein with regards to sharing information having do to with a fellow employee’s job performance or status.

I will never disclose any personal and private information about applicants for jobs with the IMACA program that I learn from their application from personal references, or their personal interview.

I will generally support and promote all IMACA programs and strive to assure that rumors or unsubstantiated or uninformed opinions aboutanywork-relatedactivityarenotdistributedinthecommunity.

Disclaimer and Consent to be Photographed

I certify that my answers are true and complete to the best of my knowledge.

I understand that false or misleading information in my application or interview may result in my release.

I hereby give Inyo Mono Advocates for Community Action, Inc(IMACA), and its successors and assignees, the right to use,reproduce and distribute mphotograph for educationalpublication or marketing purposes without any compensation to me.

I understand that IMACA shall have total and exclusive control over the use of my photographand I waive any right to inspect or approve any proposed publication in any medium. I hereby release Inyo Mono Advocates for Community Action, Inc. and its successors and assignees from any liability by virtue of my photograph.

I hereby give Inyo Mono Advocates for Community Action, Inc. the right to use quotations attributed to me for educational, promotional or marketing purposes without further compensation. I consent that all of this material shall be solely and completely the property of IMACA.

I state that I am at least 18 years of age and competent to contract in my name. (If not at least 18; must have the signature of a parent or guardian, restrictions apply) I have read and understand the above.
